If you are involved in any of the programs that DOIT sponsors in 2009, please fill out the demographic form (please only fill it out once in 2009). We need this information to report to our funders so that we can continue to get money to fund the programs that you participate in.

YAD Mini-Grant App

PURPOSE OF THIS GRANT PROGRAM:
Did you have a great idea that came out of the Youth Adult Dialogue? This grant program is meant help you get your great idea off the ground. You can apply for up to $500. This might not be all that you need, but it can be a seed to get your project started. DOIT may also be able to provide other support such as help raising additional money, advice, mentorship, technical assistance, etc. Be sure to ask for what you need. NOTE: All applicants must be youth-adult teams, with at least one person under 21 and one person 21 or older committed to working together on the project.

DEADLINE:
There is no application deadline for this program, but be sure to submit your application at least SIX WEEKS before the date on which you would like to start.

* What is the purpose of your project?
* How did your idea come out of the Youth-Adult Dialogue?
* How will your project contribute to the community?
* How many people will be involved in planning, organizing and carrying out the project? Do you already have these people lined up? If so, who are they; if not, how will you recruit them?
* How many other people will participate in it or benefit from it? How will you get them involved?
* How will you evaluate your success?
* A project plan showing the steps of the project, who will be responsible for each step and an approximate date by which each step would be completed.
* 8. A detailed breakdown of the costs of your project, including all the supplies, materials, equipment, labor, professional fees and any other costs you can think of. Research costs so that your budget is realistic. The total cost of your project may exceed $500, and if it does answer the next question.
If the total cost of your project will exceed $500, list your other sources of funding, including your own or your family’s money, fundraisers you plan to do, grants you’ll apply for, and money that you will need help raising.
